Clinical Analysis of Enterocutaneous Fistula


Abstract
The enterocutaneous fistula is one of the most serious complication of gastrointestinal surgery. Despite recent advance in nutritional support, patient monitoring and surgical intensive care as well as some improvement in surgical techniques,
most
contemporary studies of fistulas still report high mortality rates between 6% and 20%4.9.10).
The authors experienced 35 cases of enterocutaneous fistula. Which occurred as a complication of surgery from January 1986 to December 1991. We analyzed there case for the general concepts of enterocutaneous fistula.
@ES The results are summerized as follows:
@EN 1) The ratio of men and women was 1.9 : 1. The most prevalent age group was 4th decade in 15 cases(42.8%)
2) The most frequent site of enterocutaneous fistula was the large colon with 23 cases(65.7%).
3) Etiology of the all cases were postoperative complication. The underlying disease was 17 cases of appendicitis, 9 cases of malignancy and 5 cases of trauma.
4) Latent period of fistula after operation was most commonly 6 to 10 days with 18 cases(51.4%).
5) The incidence of complication of fistula was 71.4% and wound infection was the most common complication(31.3%).
6) Among diagnostic method, fistulography was the most accurate method(91.3%).
7) According to the amount of daily drainage, High output fistula were 14 cases(40%). Low output were 21 cases(60%).
8) Conservative treatment was done 22 patients and operative treatment was done 13 patients.
9) Operative procedure was done 5 cases with fistulectomy, 5 cases with bowel resection, and 1 cases with bypass procedure.
10) the overall mortality ratio was 11.4%. The most common cause of death was sepsis.
